Reporting Bias, Graffiti, Anonymous Phone Calls and Emails

You may use the online Bias/Discrimination/Harassment Reporting Form to report Bias Incidents that do not require EOAA assistance but need to be reported. An example might be graffiti or an anonymous phone call with discriminatory content. This information is very valuable in getting a picture of the entire campus, and identifying any trends. Click the link above to report online, or click here (Word) to download the Discrimination/Harassment Reporting Form (UM Form 1622) in Word document format. This form can be printed and submitted to the Office of Equal Opportunity and Affirmative Action, 419 Morrill Hall, 100 Church St. SE, Minneapolis, MN 55455; fax number: 612-624-5223; or email this information to the EO/AA Office at eoaa@umn.edu.

If the incident involves assault, damage to property, or other potentially criminal conduct, contact the University of Minnesota Police Department directly (non-emergency: 612-624-COPS [2677]; emergency: 911.) The EO/AA Office may forward the Discrimination/Harassment Reporting Form to the U of M Police if the incident appears to be a crime.
